Job Summary:

Coordinates and supports the health center program deliverables and objectives within the health delivery system to maximize its efficiency in delivering high-quality patient care. Consults and educates patients, staff and community on the health care program objectives. Collaborates with patients, families, caregivers, providers, department staff and the community at large. Supports initiatives to ensure the efficient implementation of all health center programs aimed to meet the organization's patient experience expectations which includes develops program charters and timelines, works with sites to manage and monitor activities, assist sites in researching and developing site specific solutions that adhere to system-wide workflows, coordinates creation of communication and awareness activities and collateral materials, facilitates workflow development, team tools and project task lists, and collects utilization data. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group. Coordinates small program(s) with limited budget/impact.

Job Requirements:

Education and Work Experience:

  • High School Education/GED or equivalent: Preferred
  • Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
  • Five years' relevant experience: Preferred

Facility Specific License/Certifications:

  • Hospital Fire and Life Safety (HLFS): Required

Essential Functions:

  • Supports the daily operations of one or more health care programs and coordinates program activities to meet the program's objectives. Oversees the implementation of policies and practices. Manages communications through media relations and social media. Helps build positive relations within the team and external parties. Schedules and organizes meetings/events. Supports growth and program development.
  • Supports, advocates and educates patients, families, and the community at large on the health care programs to meet patient experience expectations.
  • Collaborates and communicates with patients, physicians, community at large, other departments, and staff.
  • Reviews and adheres to safety and government regulations.
  • Performs other job-related duties as assigned.
