The Physician Assistant (PA) in the Internal Medicine Department at Premium Health is responsible for providing comprehensive care to adult patients, managing both acute and chronic conditions. This role involves conducting thorough assessments, diagnosing and treating a wide range of medical conditions, and performing procedures to support patient care. The PA works closely with the Medical Director and other healthcare providers to ensure that patients receive high-quality, personalized care. This position is integral to the success of the Internal Medicine Department and aligns with Premium Health’s mission to deliver exceptional, patient-centered, and culturally sensitive healthcare services.


This position requires Sunday availability and post 5 PM availability to serve our patients!


Responsibilities

•Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic testing, to diagnose and manage a wide range of acute and chronic medical conditions.


•Develop and implement individualized treatment plans, including prescribing medications and therapies, and adjusting treatment plans based on patient response and clinical guidelines.


•Provide ongoing care for patients with chronic conditions, including diabetes, hypertension, and cardiovascular disease, ensuring continuity of care and optimal patient outcomes.


• Collaborate with physicians, nurses, and other healthcare providers to coordinate patient care, including referrals to specialists and coordinating multidisciplinary care plans.


•Perform routine procedures, such as suturing, wound care, and joint injections, as well as assisting in more complex procedures as needed.


•Educate patients and their families on medical conditions, treatment plans, and preventive care strategies to promote overall health and wellness.


•Monitor and review patient progress through follow-up appointments, lab results, and diagnostic testing, making adjustments to treatment plans as necessary.


•Provide coverage for patient visits, including urgent care and walk-in appointments, ensuring timely and effective care for all patients.


•Maintain detailed documentation of all patient interactions, treatment plans, and clinical decisions using the electronic medical record (EMR) system.


•Address patient concerns and escalations, demonstrating a commitment to patient satisfaction and high-quality care.


•Participate in clinical meetings, case conferences, and continuing education opportunities to stay current with best practices and enhance clinical skills.


•Adhere to the highest standards of care and comply with all relevant regulatory guidelines and protocols.


Qualifications


•Active and unrestricted license as a Physician Assistant in New York State.


•Clinical experience in internal medicine or primary care.


•Proficiency in conducting patient assessments, developing and implementing treatment plans, and managing chronic conditions.


•Excellent interpersonal, written, and oral communication skills.


•Ability to exercise sound judgment and discretion in clinical decision-making.


•Ability to maintain highly confidential information in accordance with HIPAA and other regulatory requirements.


•Strong organizational and time management skills with attention to detail in patient documentation.


•Proficiency in using electronic medical records (EMR) and other healthcare technologies.
