Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois PT license required to start the assignment.
• BLS certification required from the AHA or ARC.
• Minimum 1 year of recent clinical experience in outpatient, home health, or skilled nursing settings preferred; client may require home health experience.
• Experience with documentation in electronic medical record systems preferred.
• Valid driver’s license and reliable transportation required for home health travel between patient homes.
