Job Title: Physical Therapist – Home Health
Location: Newark, Nutley, Belleville, and Bloomfield
Shift Schedule: 8:00 AM – 4:30 PM
On-Call Requirement: No on-call; weekends optional
Mileage Reimbursement: Yes (from second patient visit through last patient visit)
Job Summary
We are seeking a dedicated and skilled Physical Therapist (PT) to deliver high-quality home health physical therapy services to patients within Newark, Nutley, Belleville, and Bloomfield. This role involves comprehensive patient assessments, individualized treatment planning, and ongoing care to help patients improve mobility, strength, and overall functional independence in the comfort of their homes. The ideal candidate has strong clinical judgment, home health experience, and proficiency with electronic documentation systems.
Key Responsibilities
Perform Start of Care (SOC) visits, physical therapy evaluations, routine revisits, re-evaluations, and discharge assessments in patients’ homes.
Develop, implement, and modify individualized physical therapy care plans based on patient needs, physician orders, and clinical findings.
Provide therapeutic interventions to improve strength, balance, mobility, pain management, and functional performance.
Evaluate patient progress toward goals and adjust treatment plans as appropriate.
Educate patients and caregivers on exercises, safety, disease processes, and strategies to improve independence and prevent injury.
Accurately document all visits, assessments, and outcomes in the electronic health record system (HCHB/PointCare).
Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated, patient-centered care.
Maintain compliance with all regulatory, safety, and quality standards.
Required Qualifications
Bachelor’s degree in Physical Therapy (or higher, per state requirements).
Active and unrestricted Physical Therapist license in the applicable state.
Experience with Homecare Homebase (HCHB) and/or PointCare.
Valid driver’s license with reliable transportation.
Ability to work independently and manage a field-based schedule effectively.
Preferred Qualifications
Prior home health physical therapy experience.
Strong communication, time-management, and organizational skills.
Compassionate, patient-focused approach with strong problem-solving abilities.
Physical & Work Requirements
Ability to travel within assigned coverage areas.
Ability to lift, reposition, or assist patients as needed.
Capability to perform duties requiring standing, walking, bending, and manual therapy.
