Centra Healthcare Solutions is seeking an experienced Physical Therapist (PT) that is licensed or in the immediate process, and qualifies, of obtaining Physical Therapist (PT) licensure in the state of MA to work in the specialty area of Rehabilitation.
- This is a contract, Days position.

Minimum Requirements include:
- 1 year of recent experience as a(n) Physical Therapist (PT) specializing in Rehabilitation.
- Current Physical Therapist (PT) license within the state of practice.
Role Summary
The Physical Therapist partners with residents and the interdisciplinary care team to provide physical therapy services that maximize functional abilities and independence in activities of daily living.
Essential Duties and Responsibilities
- Supports and promotes the mission and values of Covenant Health Ministry
- Performs evaluative and therapeutic procedures under physician or approved provider orders
- Observes all precautions, indications, and contraindications during care
- Evaluates residents according to department protocols
- Develops individualized treatment plans, including:
- Short- and long-term functional goals
- Frequency and duration of therapy
- Appropriate therapeutic modalities
- Incorporates input from residents and caregivers into care plans
- Communicates with referring physicians and care team regarding treatment and discharge planning
- Participates in interdisciplinary team meetings
- Monitors patient progress and adjusts treatment plans as needed
- Documents all evaluations and treatments accurately in the Electronic Medical Record (EMR)
- Ensures documentation is clear, accessible, and compliant
- Recommends and provides training on assistive devices
- Educates residents and caregivers and documents outcomes
- Delegates appropriate tasks to Physical Therapist Assistants (PTAs)
- Collaborates with therapy team to ensure high-quality resident care
- Maintains compliance with Medicare, Medicaid, and other reimbursement guidelines
- Provides in-service education and supports staff development
- Assists in developing new rehabilitation programs and services
- Stays current with professional standards and industry trends
- Supervises assistant-level staff in accordance with state and professional guidelines
- Reports performance concerns to leadership when appropriate
- Mentors and supports onboarding of new staff
- Maintains cleanliness of clinical and office areas
- Assists with equipment maintenance and cleaning
- Follows all infection control procedures
- Maintains required licenses, certifications, and competencies
- Completes annual compliance training
- Performs other duties as assigned

Education and Experience
- Bachelor’s degree required; Master’s or Doctorate preferred
- Active Physical Therapist (PT) license in the applicable state
- Basic Life Support (BLS) certification preferred
- Prior SNF or long-term care experience required
- Equivalent combinations of education and experience may be considered
Working Conditions / Physical Demands
- Regularly required to sit, stand, walk, bend, stoop, twist, and reach
- Must be able to lift up to 50 pounds independently
- Lifts over 50 pounds require a two-person assist
- Frequently lift/carry up to 20 pounds
- Moderate stress due to responsibility for quality patient care

Weekly Pay Range = $2,300 to $2,700
