Supportive Home Health is seeking a dedicated and skilled Physical Therapist Assistant (PTA) to join our team in Los Angeles County on a Contracted/1099 basis. This role is ideal for a compassionate professional who enjoys helping patients regain mobility, independence, and quality of life through high‑quality rehabilitative care delivered directly in the home.

Qualifications

• Graduation from an APTA‑approved Physical Therapist Assistant program
• Active PTA license in the State of California
• One year of post‑licensure experience preferred
• Current CPR/BLS certification
• Ability to work independently with minimal supervision
• Strong communication skills and patience when working with adult and geriatric patients
• Reliable transportation for field visits

Position Summary

The Physical Therapist Assistant supports the supervising Physical Therapist in delivering physician‑ordered rehabilitative services. You will provide treatments, educate patients and caregivers, and ensure effective implementation of the Plan of Care (POC). Your role is essential in helping patients improve strength, mobility, safety, and overall function.

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
• Educate patients and families on home exercise programs, balance training, gait training, and safe mobility techniques
• Assist patients with therapeutic activities and use of assistive devices
• Observe, document, and report patient progress and changes to the healthcare team
• Evaluate home environments and recommend modifications to support patient safety and independence
• Maintain accurate, timely documentation in accordance with agency and regulatory standards

Compensation (Contracted/1099)
