• Paid travel time between facilities
• Benefits eligible after 30 days
• Accrued PTO up to 20 days during Year 1

About the Role

We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our skilled nursing facility team supporting residents across two locations in the Dayton/Germantown area. The PTA will work closely with Physical Therapists to help residents regain mobility, improve strength and balance, and achieve optimal physical function. This role is ideal for a clinician who enjoys working in a collaborative rehabilitation environment and is passionate about improving patient outcomes in a Skilled Nursing Facility (SNF) setting.

Key Responsibilities

• Assist Physical Therapists with implementing individualized treatment plans and rehabilitation goals
• Support residents with mobility, strengthening, balance, and therapeutic exercises
• Monitor patient progress and communicate updates to the therapy team
• Educate patients and caregivers on exercises, therapy techniques, and post-discharge maintenance programs
• Help residents maintain achieved functional levels through education and activity modification
• Maintain accurate documentation and EMR records in compliance with facility standards
• Collaborate with interdisciplinary teams to ensure quality patient care

Qualifications

Required:

• Associate’s Degree from an accredited Physical Therapist Assistant program
• Current, active, and unencumbered PTA license in the state of Ohio
• Basic computer proficiency and ability to navigate EMR and service delivery software
• Strong communication and patient care skills

Preferred:

• Prior Skilled Nursing Facility (SNF) or rehabilitation experience
• Vent/trach experience or knowledge is a plus
