Job Summary
Functions competently as a clinical pharmacist in the provision of pharmaceutical care. Prisma Health service standards will be consistently demonstrated while providing care to all the patient populations serviced at Prisma Health.Essential Functions

Provides competent service in the medication use process as related to prescribing, administration and monitoring with an overall emphasis on the provision of most efficacious, safe and cost-effective therapy.This will be accomplished through the provision of accurate up to date pharmacotherapy management to all patient populations serviced.Activities include, but are not limited to, participation in patient care rounds, individual patient care assessment (patient specific problem list, medication profile and administration record review), development of individual pharmacotherapy plans (selection of drug therapy, monitoring plans) and clinical consults and committee work.
Provides accurate and relevant informal and formal drug information in a timely manner to all customers serviced by the pharmacy department.Drug information provided is in the form of written responses, in-service education, committee participation or verbal communication with health care providers and is documented.
Provides competent service in the medication use process related to dispensing. This function will be accomplished through competent interpretation, review, recording of medication ordered and accurate dispensing of medication to patients and patient care areas with an emphasis on safety, efficacy, state and federal regulations and hospital policy.
Identifies opportunities for improvement within the department of pharmacy and the medication use process and develops a plan to implement and measure change.This function will be accomplished by the implementation of formal process improvement initiatives, research projects, or other projects as identified or assigned.
Oversees and directs works of pharmacy students, technicians and interns where appropriate.
Performs other duties as assigned.
Supervisory/Management Responsibility
This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
Education - Master's degree or other Advanced Training as a Pharm D
Experience - Acute Care Hospital Pharmacy experience preferred.
In Lieu Of
In lieu of an active Pharmacist license, may be a graduate of an ACPE accredited US college of Pharmacy with an intern certificate pending initial licensure.
Team members employed in this job prior to July 1, 2020, are grandfathered under prior educational and experience requirements. In addition, team members must be certification eligible and will have one year to obtain required certification.
Required Certifications, Registrations, Licenses
RPh-South Carolina Pharmacist license required for team members working in South Carolina.
RPh-Tennessee Pharmacist license required for team members working in TN within 90 days of start date.

Distributive and Clinical Pharmacy Practice skills preferred
