Job Title: Clinical Pharmacist

Job Description

The Clinical Pharmacist is an integral part of the interdisciplinary team, dedicated to preventing and curing cancer through safe, evidence-based, cost-effective, and patient-centered medication management. This role involves supporting physicians, nurses, pharmacists, and other staff in both hospital and ambulatory clinic settings. The Clinical Pharmacist leverages extensive clinical experience to design, implement, monitor, and modify pharmaceutical care plans, optimizing patient outcomes.

Responsibilities

  • Oversee the safe and appropriate use of medication prescribing, dispensing, administering, and monitoring.
  • Provide clinical pharmaceutical care to optimize drug therapy for cancer patients by recommending, implementing, monitoring, and modifying individualized treatment plans in collaboration with the healthcare team.
  • Review, clarify, and verify medication orders to ensure completeness and appropriateness.
  • Supervise the accurate preparation and distribution of hazardous and non-hazardous drugs per policies and procedures.
  • Process and verify chemotherapy orders, conducting double checks as necessary.
  • Develop, interpret, and provide medication-related information and education for pharmacists, providers, nurses, and other healthcare personnel.
  • Coordinate and provide medication therapy education and counseling to patients and caregivers.
  • Precept pharmacy students, residents, and others in training.
  • Participate in clinical research and projects, providing leadership for developing, implementing, and maintaining medication-related guidelines, policies, and standards.
  • Manage the medication use process to improve workflow, quality, and safety for patients and staff.
  • Oversee the cost-effective use of medications to ensure appropriate utilization of resources and reduce financial risk.

Essential Skills

  • Doctor of Pharmacy Degree
  • Board of Pharmacy Specialties (BPS) certification
  • Extensive experience in malignant hematology and oncology pharmacy practice
  • Proficiency in medication management and interdisciplinary team collaboration
  • Experience in formulary management and guideline development
  • Expertise in policy development and implementation
  • Knowledge of oncology reimbursement and coverage
  • Strong clinical research skills

Additional Skills & Qualifications

  • PGY-2 Residency in Hematology/Oncology
  • Experience in precepting pharmacy students and residents
  • Proficiency in medication therapy education and counseling
  • Competency in developing medication-related guidelines and policies

This is a Permanent position based out of Tampa, FL.

Pay and Benefits

The pay range for this position is $120000.00 - $180000.00/yr.
