Job Summary:

Provides comprehensive pharmaceutical care to outpatient clinic patients to ensure safe and effective drug therapy, taking into consideration patient’s diagnosis, prescribed treatment, age group and other identified needs. Uses independent decision-making, clinical judgment, and in-depth knowledge of pharmacotherapy and therapeutics.

Job Requirements:

Education and Work Experience:

  • Graduate of a school of pharmacy recognized by State of California Board of Pharmacy: Required
  • Graduate of a school of pharmacy recognized by State of Oregon Board of Pharmacy: Required
  • Bachelor's Degree: Preferred
  • One years' experience in a specialty pharmacy setting: Preferred
  • Two years' experience in a retail pharmacy setting: Preferred

Licenses/Certifications:

  • Pharmacist license in the state of practice: Required
  • Registered Pharmacist licensure in state of practice: Required
  • Board Certified Pharmacotherapy Specialists (BCPS) or equivalent, or three years' experience in ambulatory care: Preferred

Essential Functions:

  • Coordinates clinical pharmacy applications to patient care. Reviews medication utilization to establish the best therapeutic outcomes. Assures that medication prescribing is optimal with respect to indication, dose, route, frequency, history of allergy, and does not duplicate or interact with the effects of other drugs. Exercises independent judgment on complex Pharmacy cases. Provides direct patient care pursuant to collaborative agreements specific to the practice setting.
  • Provides medication consults on drug therapy and drug dosages using a high level of technical skill and critical thinking ability.
  • Provides consultant services to various medical staff committees and demonstrates clinical knowledge and skill regarding medications used in the care of newborn, infant, toddler, child, adolescent, adult and geriatric patients.
  • Maintains controlled substance records according to established procedures. Assists and audits regulatory compliance.
  • Participates in training programs for pharmacists and other ancillary hospital personnel. Mentors and trains staff as needed.
  • Performs other job-related duties as assigned.
