About the Role:The Peritoneal Dialysis & Home Hemodialysis Coordinator is a highly skilled Registered Nurse responsible for overseeing all aspects of home dialysis programs while supporting clinical operations within nephrology. This role combines direct patient care, care coordination, and clinical leadership to ensure safe, efficient, and high-quality outcomes for a medically complex patient population.
This position plays a critical role in maintaining program stability, supporting growth, and advancing value-based care initiatives through proactive patient management and strong interdisciplinary collaboration.
Primary Responsibilities:Serve as the primary clinical coordinator for patients receiving peritoneal dialysis and home hemodialysis, ensuring safe, timely, and comprehensive care. Conduct patient assessments, provide education and training for home dialysis therapies, and monitor patient progress, adherence, and outcomes, escalating concerns as appropriate. Coordinate labs, prescriptions, supplies, and follow-up care, while supporting transitions such as hospital discharges, new therapy starts, and modality changes.
Provide day-to-day guidance and support to nephrology clinical staff, promoting consistent workflows, adherence to clinical standards, and a high level of patient care. Serve as a clinical resource, supporting onboarding, training, and skill development across the team.
Partner with leadership to support program development, including workflow design, process improvement, and expansion of home dialysis services. Ensure compliance with regulatory requirements, quality standards, and documentation practices, while participating in quality improvement initiatives and performance tracking.
Collaborate closely with nephrologists, advanced practice providers, nurses, technicians, and external partners to ensure coordinated, patient-centered care. Support value-based care initiatives by identifying risks, reducing barriers to care, and promoting home-based therapies that improve outcomes and reduce hospital utilization. Provide ongoing patient and caregiver education, reinforcing safe practices, troubleshooting, and long-term therapy success.
