Responsibilities
Under the Direct Supervision of the VP of Physician Services, the Patient Navigator willwork in collaboration and continuous partnership with patients and their family/caregiver(s),clinic/hospital/specialty providers and staff, and community resources to connect patients to relevantcommunity resources, with the goal of enhancing patient health, well-being, decreasing patientreadmissions and increasing patient satisfaction.
Qualifications
MINIMUM EDUCATION:
High School Graduate or GED Equivalent; relevant experience in lieu of High School Diploma/GED education will be considered.
MINIMUM WORK EXPERIENCE:
3 years previous customer service experience.

REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES:
Previous experience in a customer support role. Strong phone andverbal communication skills along with active listening. Customer focusand adaptability to different personality types. Ability to multi-task,priorities, and manage time effectively. Ability to establish and maintainstrong working relationships with staff and management; excellentorganizational skills with emphasis on detail and ability to effectivelycommunicate both verbally and in writing. Detailed oriented, teamplayer, and an independent thinker.
