About the Role

As a Palliative Care Nurse Practitioner, you’ll provide palliative care consultations for patients with serious illness who are not yet ready or eligible for hospice. You’ll deliver holistic, patient‑ and family‑centered care across physical, emotional, psychosocial, and spiritual needs.


What You’ll Do

  • Provide palliative care consultations for patients and families
  • Complete comprehensive clinical, psychosocial, and spiritual assessments
  • Develop and manage individualized plans of care
  • Perform medication reconciliation and ongoing care evaluation
  • Practice evidence‑based, ethical palliative medicine
  • Collaborate closely with caregivers and interdisciplinary teams


Required Qualifications

  • Active RN/APRN license in Washington
  • Nurse Practitioner or CNS with prescriptive authority
  • BCLS and current CPR certification
  • 5+ years of clinical nursing experience
  • Valid driver’s license and reliable transportation


Preferred Qualifications

  • 1+ year as an APRN in Hospice, Palliative Care, or Oncology
  • Advanced practice palliative care certification (ACHPN) or willingness to obtain
