The Integrated Behavioral Health Consultant will act as an independent clinician and will be consulted by physicians to offer assistance to patients when habits, behaviors, stress, worry, or emotional concerns about physical or other life problems are interfering with daily life and/or overall health. Highlights of the position include the opportunity to work closely with medical professionals including PCPs and psychiatrists while independently diagnosing and treating patients using evidence-based psychosocial interventions such as CBT, motivational interviewing, behavioral activation, problem-solving, and distress tolerance. This position offers a gratifying blend of both therapist and case manager duties, and is engaging, dynamic, team-based, challenging, and community-oriented. Ambulatory Care at Penrose Home Health services a 60-mile radius from our office here in Colorado Springs. Our interdisciplinary team includes skilled nursing, physical/occupational/speech therapy, social work and CNAs. We care for a diverse population of patients, aged 18 and above and deliver skilled care in the patient's home environment. Skilled nursing addresses a wide variety of patient education such as medications and disease processes, as well as direct skills including wound care, ostomy teaching and IV/PICC infusion. We strive to deliver exceptional care and educate patients on how to manage their health conditions at home. The flexibility in home health is second to none. Our clinicians self-schedule their caseload, working around their own and the patient's schedule.
