Assignment Responsibilities
As the Occupational Therapist in Home Health you will assess and evaluate patient needs related to functional status, activities of daily living, fine motor coordination, and home safety. You will conduct home assessments, recommend adaptive equipment, document CMS-required assessments, coordinate with physicians and interdisciplinary team members, and record patient responses to treatment. The role requires adherence to state scope of practice and use of evidence-based interventions.

Maintain professional appearance with neat, clean scrubs. Demonstrate strong communication and teamwork. Follow all agency and regulatory documentation requirements. Travel between patient homes within the assigned coverage area and document mileage per policy.
