Duration: 3 months with contract extensions
Shift: Days, Mon-Fri

Coverage Areas: I need 2 positions; 1 for Milford CT and surrounding towns and 1 for Waterbury CT and surrounding towns
Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes

Roles & Responsibilities
- Provide skilled home health services to patients in their homes per physician orders and plan of care
- Perform patient assessments and deliver safe, effective, compliant care
- Monitor patient progress and report changes to the care team
- Educate patients and caregivers on treatment plans and disease management
- Coordinate care with physicians and interdisciplinary team members
- Maintain accurate, timely documentation per LHC Group and regulatory standards
- Travel within assigned coverage area; mileage reimbursed between visits
Requirements
- Home Care/Home Health experience (required)
- Active Connecticut license (role-dependent)
- CPR/BLS certification
- Valid driver’s license and reliable transportation
Must have qualifications/experience: Home Care experience
Dress code/scrub color: Business casual or scrubs
