Schedule: 5 days per week, 8:00 AM – 4:30 PM (40 hours available)

Job Summary
We are seeking a licensed and experienced Occupational Therapist (OT) to provide high‑quality home health services across the Bourne Territories of Massachusetts. This role delivers patient‑centered occupational therapy in community and assisted living settings, supporting patients in achieving greater independence, safety, and functional performance. The ideal candidate has strong home health experience, excellent clinical judgment, and a collaborative approach to care.
Key Responsibilities
Perform Start of Care (SOC) visits, occupational therapy evaluations, recertifications, discharges, and ongoing treatment visits.
Develop, implement, and update individualized occupational therapy plans of care in alignment with physician orders and patient goals.
Provide skilled OT interventions to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Deliver occupational therapy services in private homes, community settings, and assisted living facilities.
Assess patient safety, cognitive function, and functional performance within the home environment.
Educate patients and caregivers on home safety, adaptive equipment, compensatory strategies, and therapeutic activities.
Coordinate care with physicians, nurses, and the interdisciplinary care team to ensure continuity and quality of care.
Accurately document evaluations, treatments, and outcomes in Homecare Homebase (HCHB) in accordance with regulatory and organizational standards.
Maintain compliance with all federal, state, and agency policies and quality requirements.
Required Qualifications
Active and unrestricted Occupational Therapist (OT) license in the state of Massachusetts.
Prior home health experience.
Experience using Homecare Homebase (HCHB).
Valid driver’s license with reliable transportation.
Ability to work independently in a field‑based environment.
Preferred Qualifications
Experience providing cognitive therapy.
Strong clinical documentation and assessment skills.
Experience working with geriatric populations and assisted living settings.

Work Environment & Travel Requirements

Daily travel within assigned coverage areas is required.
Travel between multiple patient locations during a single shift is expected.
