The Nursing Outcomes Improvement Facilitator - Admin RN leads interprofessional efforts to implement evidence-based practice standards that advance nursing practice and improve patient outcomes. Responsible for developing nurse leaders through mentorship of nursing colleagues and role modeling of professional practice. Is a resource person and consultant for initiatives involving nursing colleague development, patient care, and nursing practice. Plays a key role in efforts to Pathways designation and support a shared decision-making philosophy. Operationalizes duties and responsibilities in a manner that supports the goals of TMH and the Nursing Division. Embodies the Memorial Health Values of Safety, Integrity, Quality, and Stewardship that support our mission and vision.

Responsibilities
Promote evidence-based practice among nursing colleagues.Observes current nursing practice and compares with recognized professional standards to determine improvement opportunities requiring process changes or educational intervention.Collects and analyzes data to determine necessary process changes and developmental needs of nursing personnel. Develops and leads follow-up interventions to address needs.Facilitates the development of critical thinking skills of nursing colleagues through ongoing consultation, role modeling, and case reviews.Facilitates implementation of Professional Practice Model and Lean Six Sigma tools to promote efficient and effective nursing practice and optimal patient outcomes.Collaborates with interprofessional colleagues and physicians in performance improvement.Facilitates achievement of Nursing Division quality improvement goals.Leads (or co-leads) housewide nursing councils, committees and task forces focused on nursing practice and patient outcomes improvement.Participates in Unit-Based Councils on a regular basis to promote process improvement and sustain practice changes.Participates in research, publication, and poster presentations to generate and disseminate new nursing knowledge and innovations in practice.Promote the transition process of nursing employees.Serves as a mentor for nursing colleagues, providing for individualized counseling and guidance as needed.Serves as a clinical role model to colleagues.Serves as a resource person and consultant for nursing colleagues to promote effective transition processes.Communicates concerns about ineffective transition processes to the appropriate nurse manager.The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job.
