Hennepin Healthcare has a flexible part-time to full time (flexible up to 1.0 FTE) opportunity for aPhysician Assistant or Nurse Practitioner to work in Jail Health Services Medical Clinic at Hennepin County Adult Detention Center.(Employees working at .5 FTE and above are eligible for full medical/dental benefits, as well as several additional prorated benefits if less than full-time.)Hours are primarily daytime weekday shifts but also include some remote on-call duties during evening and night shifts.Exact schedule TBD based on clinical needs and provider preference.
This position is responsible for evaluation and treatment of patients with acute and chronic physical, mental health, and substance use conditions, medication reconciliation & management, and patient education & counseling within scope of practice and in collaboration with the supervising/collaborating physician.

RESPONSIBILITIES
Provide direct medical care to adult patients as part of an interdisciplinary team of nurses, mental health providers, social workers, and correctional staff in the jail setting
Perform initial and ongoing assessment of patients' physical, mental health, and substance use conditions
Obtain and evaluate health and medical history, perform physical exam, conduct preventive screening procedures based on age and history, identify medical and health risks
Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
Formulate appropriate differential diagnosis based on history, and physical examination
Identify needs of the individual and develop a plan of care and/or education plan
Order, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and medications
Make appropriate referrals to other health professionals and community agencies
Reassess and modify the plan as necessary to achieve medical and health goals
Contribute to ongoing medical education teaching programs
Support new and existing programs which expand the organization’s role in the community
Participate in performance improvement activities
Adhere to template and productivity standards established by the department
Work closely with interdisciplinary teams to provide the best care for the population served
Respond timely to in-basket, forms, email, and paging communication regarding patient care
Demonstrate respect and regard for the dignity of all patients, visitors, and fellow employees to ensure a professional, responsible, and courteous environment
Perform other related duties but only after appropriate training
