Position Overview:


As a Nurse Practitioner, you will play a pivotal role in providing primary care services to patients, collaborating closely with our Medical Director and Director of Medical Practice Operations. The Nurse Practitioner role is a vital position in the Monadnock Health Partners (MHP) network of primary care practices. Responsibilities:


Patient Care Responsibilities:


  • Patient-Centered Care: Conduct thorough assessments and collaborate on personalized care plans.
  • Comprehensive Treatment: Diagnose and treat illnesses, prescribing medications as needed.
  • Preventive Healthcare: Deliver preventive care services like screenings and education.


Administrative Duties:


  • Health Record Management: Maintain accurate and compliant electronic health records.
  • Teamwork & Communication: Collaborate with healthcare professionals for optimal patient outcomes.
  • Quality Improvement: Participate in initiatives to enhance patient care practices.

Qualifications:


  • Prefer five years Internal Medicine experience in clinical or practice setting.
  • Must be a graduate of an accredited school of nursing, and must have successfully completed an approved nurse practitioner program.
  • Must have a current license to practice as an ARNP in the State of New Hampshire.
  • Must have a current DEA number, and must be insurable through MCH professional liability insurance coverage.
