Advanced Cardiac Therapies (ACT) / Acute Cardiac Care Unit (ACCU) Nurse Practitioner or Physician AssistantPosition SummaryWe are seeking a Nurse Practitioner or Physician Assistant to join a hybrid role across the Advanced Cardiac Therapies (ACT) Team and the Acute Cardiac Care Unit (ACCU). This position supports the care of pediatric and adult patients with cardiomyopathy, heart failure, congenital heart disease, arrhythmias, and pre- and post-transplant needs across inpatient and outpatient settings.Key Responsibilities for both ACT and ACCU:• Partner with Physicians, Nurse Practitioners, staff nurses, administrative support staff, and multidisciplinary team members across inpatient units, Cardiac ICU, cardiac catheterization suite, and outpatient clinic.• Responsibilities include inpatient rounds, team meetings, documentation, prescriptions, prior authorizations, referrals, education, and discharge planning.• Provide care for pediatric and adult patients with cardiac conditions including congenital heart disease, arrhythmia, heart failure, pre and post procedures and all phases of heart transplant management• Collaborate with multidisciplinary teams to ensure optimal patient outcomes and continuity of care.• Work collaboratively in a family-centered environment.• Support complex discharge planning and transitions of care.ACT:• Support longitudinal care coordination, including frequent communication with patients and families for home care needs, clinic sessions, billing, and regulatory requirements.• Opportunity for cross coverage of lung transplant patients.ACCU:• Coordinate care for patients admitted for cardiac and non-cardiac medical issues in collaboration with consulting services.Minimal QualificationsEducation• Master of Science in Nursing or Master of Science in Physician Assistant StudiesExperience • 1-2 years of Advanced Practice experience with complex medical care preferred
• 2-3 years of pediatric RN experience required for Nurse Practitioners, cardiac nursing experience preferred
• 1+ year of NP or PA experience preferred
• Experience with pediatric complex care or outpatient clinics preferred
• Pediatric cardiology or transplant experience preferred
• Strong communication skills, flexibility, and ability to work in a team environment
• Strong organization, written/verbal skills, and EMR competency requiredLicensure/Certifications• Current MA PA license or MA NP with PNP or FNP certification
• BLS and PALS certification required
• Willingness to pursue Transplant Coordinator certificationSchedule• 40 hours per week: 20 hours ACT / 20 hours ACCU• ACCU Schedule:
– 6-week block: 4 weeks of two 12-hour shifts/week; 2 weeks of one 12-hour shift/week• ACT Schedule:
– 2 ten-hour shifts per week• Additional requirements:
– Up to every 3rd weekend
– Up to 25% nights
– Some holidays requiredThis role offers a competitive sign-on bonus ranging from: $5,000 to $20,000, based on your experience.
