This Advanced Practice Provider role offers a unique opportunity to work within a highly specialized transplant center caring for patients across all organ types, where clinical complexity and meaningful impact go hand in hand. We are seeking a highly motivated and clinically astute Nurse Practitioner or Physician Assistant to join our Transplant Infectious Disease team, providing comprehensive outpatient infectious disease management for solid organ transplant recipients. Key Responsibilities

  • Provide specialized outpatient care to solid organ transplant recipients with complex infectious disease needs
  • Evaluate, diagnose, and manage a broad range of pre‑ and post‑transplant infectious complications across all organ types
  • Collaborate closely with transplant physicians, surgeons, pharmacists, and interdisciplinary team members to develop and implement individualized care plans
  • Monitor patients longitudinally to ensure continuity, optimization, and quality of care
  • Participate in patient and caregiver education with emphasis on infection prevention and management strategies
  • Apply evidence‑based practices and current clinical guidelines in transplant infectious disease care
  • Document patient encounters accurately and in a timely manner within the electronic medical record
  • Engage in quality improvement initiatives and support program development as appropriate
