Guides CHF patients through inpatient, discharge, and outpatient phases of care to ensure continuity and timely access to needed services.
Serves as the central point of contact and liaison between patients, families, and members of the multidisciplinary HVI team.
Provides education on CHF disease processes, medications, lifestyle modifications, symptom management, and follow-up care.
Engages patients in shared decision making in alignment with American College of Cardiology (ACC) guidelines and supports adherence to Guideline Directed Medical Therapy (GDMT).
Participates in clinical discussions, HVI conferences, and coordination activities that support integrated patient care.
Builds and maintains effective working relationships with physicians, APPs, nurses, and other care providers.
Assists with data collection for registries, performance monitoring, and quality improvement initiatives related to CHF outcomes and care processes.
Contributes to efforts aimed at improving patient experience and clinical effectiveness within the HVI Service Line.
Maintains UpToDate knowledge of CHF therapies, treatment guidelines, and navigation best practices.
Demonstrates competency in care coordination, patient education, communication, and professionalism.
Completes all required annual education and training.
Represents McLeod Health and the HVI Service Line professionally in both internal and community settings.
Participates in community outreach events as requested.
Performs other duties as required.
Qualifications /Training:
A minimum of 3 years of recent cardiovascular experience.
Strong knowledge of heart failure pathophysiology, GDMT medications, and other evidence-based guidelines.
Excellent communication, patient education, and care-coordination skills.
Ability to work independently and collaboratively across multidisciplinary teams.
Licenses/Certifications/Registrations/Education:
Current South Carolina (or compact state) licensure as RN (SCLLR) in good standing required.
Basic Life Support (BLS) certification required.
Advanced Certification as approved by VP of Cardiovascular Services within 3 years of hire
