Under the general direction of the Supervisor/Manager of Clinic Operations with clinical oversight from the Director of Clinical Operations, RN, the nurse navigator coordinates care across the continuum for complex patients within a specific specialty population (disease-centric) and facilitates communication between the primary physician, nursing staff, consulting physician, ancillary departments, and community resources.  Using motivational interviewing and trauma-informed care, the nurse navigator is responsible for developing authentic healing relationships with patients, providing education and information to the patient and care partner, providing support for navigating complex healthcare systems and disease pathways, providing self-management support, assessing, and improving social determinants of health, and monitoring outcomes.

Required Qualifications:

  • Graduation from an accredited RN program.
  • Upon hire: Washington Registered Nurse License 
  • Upon hire: National Provider BLS - American Heart Association 

Preferred Qualifications:

  • Bachelor’s Degree in Nursing
  • 3 years experience in disease process/specialty of focus
  • 2 years ambulatory nursing experience or care management experience
