How you’ll make an impact in this role
  • Improves patient safety by collecting comprehensive and accurate medication histories at admission, reducing the risk of medication discrepancies, adverse drug events, and preventable harm during care transitions.
  • Supports clinical teams by freeing up physicians and nurses from time-intensive medication reconciliation tasks, allowing them to dedicate more attention to direct patient care and acute clinical decision-making.
  • Reduces hospital costs and readmission rates by identifying and resolving medication errors early in the admission process, minimizing the downstream clinical and financial impact of drug-related complications.Strengthens care coordination across departments by serving as a dedicated liaison between patients, caregivers, pharmacies, and clinical staff to ensure accurate and up-to-date medication information is available at every stage of the patient’s hospital stay.
What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Pharmacy Tech credentialed from the Pharmacy Technician Certification Board (PTCB) preferred.
  • Pharmacy Tech credentialed from the National Healthcareer Association preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
What additional preferences we're seeking
  • Ability to work independently, stay focused and organized, and manage multiple tasks in a fast-paced Emergency Department environment with shifting priorities.
  • Ability to accurately record drug names, dosages, routes, and frequencies.
  • Experience identifying medication discrepancies and catching errors before they affect patient care.
  • Comfortable speaking with patients, family members, and caregivers in stressful or emotionally sensitive situations.
  • Willing to reach out to outside pharmacies, care facilities, or doctor's offices to confirm medication information.
  • Dedicated to patient safety and able to communicate clearly with patients who may be confused or have difficulty recalling their medications.
