Medi Home Health Agency, a proud member of the Medical Services of America family, is currently seeking an experienced Full-Time Hospice Social Worker / MSW for our Hospice location in Glen Allen and Richmond, VA.

What You'll Do:

· Provides social work services in accordance with the physician’s plan of treatment.

· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.

· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.

· Serves as a liaison with other community resources.

Job Requirements

· Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.

· Currently licensed to practice as a social worker in the state of VA.

· Minimum of one-year Hospice experience preferred.

· Valid Driver's License.
