General Position Description: Responsible for ensuring all ESBF member medical records are up to date and conform to industry compliance standards.


Essential Duties and Responsibilities:

  • Maintains, organizes, and updates member health records in accordance with facility policies and regulatory requirements.
  • Maintains logs, tracking systems, and databases related to medical records operations.
  • Ensures accuracy, completeness, and proper filing of all medical record documentation. Reviews medical records for errors or inconsistencies and escalate issues as needed. Assists in maintaining data integrity throughout the medical record system.
  • Processes requests for medical records from members, providers, and other allowable entities. Tracks and documents all disclosures in accordance with legal and organizational policies.
  • Verifies authorization forms and ensure proper identify verification before releasing information.
  • Protects member privacy by adhering to HIPPA regulations and organizational confidentiality standards.
  • Supports onboarding and training of new staff in medical records procedures when needed. Provides ongoing training and support to staff related to releases of information and medical record organization as needed.
  • Collaborates with clinical and administrative staff to resolve any documentation issues that arise.
  • Conducts routine and targeted audits of clinical charts to ensure documentation is complete, accurate, and compliant with regulatory, payer, and organizational standards. Tracks audit findings, trends, and error patterns to support quality improvement initiatives. Present data on audit findings and trends to department leadership as requested and when needed.
  • Assists with other administrative or EHR related tasks.
  • Participates in the process of ongoing professional development and meetings. Maintains comprehensive communication among all contacts within the agency, members, and external agencies. Models professional standards and ethics in accordance with EBF Code of Conduct, Strategic Plan, Mission, and Vision.
  • Performs other duties as assigned.

Requirements

Minimum Requirements: High School Diploma / GED with 3 years of administrative experience required. Bachelor’s degree with behavioral health experience preferred. Bilingual (Spanish) preferred.


Regulatory

  • Must be at least 21 years of age.
  • Current, unrestricted AZ driver’s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.
  • Transportation to attend trainings and perform other work-related activities. Organizational vehicles may not be available at all times.
  • Must be able to pass a criminal background check prior to employment and regular checks for the duration of the employment.
  • Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card (employer provides).
  • Ability to obtain and maintain CPR and First Aid certification (Employer provides).

Must be self-directed and be sensitive to cultural and linguistic diversity. Excellent customer service in stressful situations. Maintains professional in appearance, communications, and actions. Excellent interpersonal communication skills. Basic knowledge of confidentiality, HIPAA, policies, practices, and procedures. Possess a high level of computer proficiency. Able to organize, prioritize, and work on multiple requests. Able to adapt and adjust to changes.


Working Conditions/Physical Requirements (with or without accommodation): Normal office environment with multiple interruptions in person and through computer and phone. Interacts with employees and members of the public on a daily basis. May lift normal office equipment and materials up to 25lbs. Visual acuity to read information from computer screens and forms. Able to speak clearly in conversations and general communications. Hearing ability for communication in person, phone, and/or other electronic methods. Manual dexterity for typing and writing. Able to stoop, squat, reach, pull, push, stretch, ascend and descend stairs, stand and sit for long periods of time. May be required to work additional hours or days depending on circumstances.
