Overview
Based within the Central Health Navigation Center, the LVN Navigator Lead oversees and supports the Central Health Transitions of Care programs and additional services. This role supports a team of Medical Assistants (MAs) and Licensed Vocational Nurses (LVNs), serving as the primary contact for patients, healthcare providers, and community partners. The LVN Navigator Team Lead ensures a seamless transition for patients discharging from hospitals or other acute care facilities to post-acute care settings. Responsibilities include managing referrals, supporting discharge planning, and providing telephonic assessments, coaching and support to facilitate comprehensive healthcare system navigation. The Lead ensures proper referral transcription and navigation to the appropriate service lines. Additionally, this role involves assisting patients through inbound and outbound calls, guiding them through the complexities of the healthcare system.
Responsibilities
JOB FUNCTIONS
Essential Functions:
- Oversee a team of MAs and LVNs, providing guidance, support, and direction to provide positive patient outcomes and experiences.
- Provide inbound and outbound call support to patients, assisting them in navigating the healthcare system and addressing any concerns.
- Initially addressing any patient care coordination concerns and escalating when necessary.
- Address performances issues promptly and fairly, escalating to Nurse Management.
- Assist with development of training programs/plans for new and existing team members to enhance their skills in patient navigation and care coordination.
- Assist hospital staff, community partners, and Central Health Transitions of Care teams in navigating patients recently discharged from hospitals or other acute care facilities by securing appropriate medication care and other healthcare related assistance for discharged patients.
- Handle incoming referrals for transitions of care from internal and community partners needing access to postacute services such as Transitional Dialysis, Case Management, education programs ensuring accurate transcription and navigation to appropriate service lines.
- Oversees support of the diagnostic workqueue; ensuring diagnostic results are routed to the provider.
- Collaborate with the Central Health Transitions of Care program to improve patient outcomes and streamline processes.
- Attends the daily huddles and monthly staff meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the Navigation Center.
- Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards and to identify opportunities to improve patient outcomes.

Qualifications
QUALIFICATIONS
Education:
High School Diploma or equivalent (higher degree accepted) -Required
Graduated from an accredited License School of Vocational Nursing (LVN) -Required
Work Experience:
5 years working in a community-based organization, ambulatory clinic, home health, or hospital setting -Required
2 years working as a LVN Navigator or Intake Referral Coordinator, or in a similar role supporting patients navigating the healthcare system. -Required
2 years working in a Supervisory or Lead role in an ambulatory care setting -Preferred
Licenses and Certifications:
LVN - Licensed Vocational Nurse Must have a current, Texas or compact state license Upon Hire -Required
Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment. Upon Hire -Required
