As an LPN Central Admissions, you will facilitate the timely and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for all referred patients. While you may not closely interact with patients on the job, your work will directly impact their care and make a lasting difference in their lives. A caregiver in this position works days from 9:00AM – 5:30PM. This is a hybrid position requiring training on site in Independence, OH for about 4-6 weeks and frequent work on site.
A caregiver who excels in this role will:
Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.
Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate.
Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.
Serve as the CCHCS information specialist.
Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited Practical Nursing Program
Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
Preferred qualifications for the ideal future caregiver include:
One year of clinical experience in Homecare
Physical Requirements:
Sitting, standing and walking for extended periods of time
Manual dexterity to perform work on a PC
Good visual acuity through normal or corrected vision
Ability to hear typical conversation levels through normal or corrected hearing
Medium Work, exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects
Physical Demand requirements in excess of those for Light Work
Personal Protective Equipment:
Follow standard precautions using personal protective equipment as required
