It has a short-term rehabilitation rating of As Expected and a long-term care rating of High Performing. It is a very small facility with 25 beds and has non profit - corporation ownership. It is part of the Presbyterian Communities of South Carolina - Florence continuing care retirement community. It participates in Medicare.

What You'll Do as LPN:

• Provide compassionate, individualized care according to residents' care plans
• Perform regular rounds and monitor residents’ conditions throughout the shift
• Supervise and direct other care staff
• Communicate effectively with residents and families about care updates
• Administer medications and treatments per physician orders
• Complete documentation accurately and in compliance with regulations
• Notify physicians and families of significant resident condition changes
• Assist with resident admissions, discharges, and transfers
• Maintain clinical competency and uphold professional standards
• Able to work weekends, holidays, and according to resident needs

What We're Looking For:

• Valid LPN license in the state of South Carolina
• 1–2 years of experience in a healthcare setting preferred
• Current CPR certification
• Proficiency with basic computer skills
