Behavioral Health Clinician (Bilingual Spanish Required)

Schedule: Monday – Friday, 9:00 AM – 5:00 PM

Position Overview

We are seeking a skilled and compassionate Behavioral Health Clinician to join a collaborative, integrated care team within a primary care setting. This role focuses on delivering brief, evidence-based behavioral health interventions while working closely with medical providers to improve patient outcomes. The ideal candidate is experienced in managing co-occurring behavioral and physical health conditions and is passionate about serving diverse and underserved populations.


Key Responsibilities

  • Deliver brief, evidence-based behavioral health interventions, including motivational interviewing, problem-solving therapy, and behavioral activation within a primary care setting
  • Collaborate with primary care providers and psychiatric consultants to support comprehensive patient care
  • Monitor medication adherence, assess side effects, and evaluate treatment effectiveness
  • Participate in regular case consultations to develop individualized care plans for patients with complex needs
  • Accurately document patient interactions, treatment plans, and outcomes in electronic medical records and maintain registry data
  • Facilitate referrals to specialty services and community-based resources as needed
  • Promote patient engagement, continuity of care, and adherence to treatment plans
  • Develop and implement relapse prevention strategies for patients in remission


Required Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, or a related behavioral health field
  • Active licensure as one of the following: LCSW, LMHC, LPC, LMFT, Psychologist, or Nurse Practitioner (NP)
  • Minimum of 3 years of experience supporting patients with co-occurring behavioral health and medical conditions
  • Bilingual in Spanish and English (required)
  • Familiarity with electronic medical records (EMR) systems and clinical registries
  • Strong understanding of healthcare systems, chronic disease management, and social service programs


Preferred Qualifications

  • Experience working in collaborative care or integrated primary care models
  • Background in care coordination, case management, or population health initiatives
  • Experience working with diverse patient populations and addressing social determinants of health
  • Strong organizational and time management skills with the ability to manage multiple patients and priorities
  • Ability to mentor or support junior staff and contribute to clinical best practices
