We are seeking a qualified healthcare professional for a 3-month hybrid assignment focused on care management. This role is ideal for candidates with experience supporting patients with HIV, substance use disorders, and mental health conditions. The position is primarily non-patient facing and involves care coordination, case management, and interdisciplinary collaboration.
Key Responsibilities
- Coordinate and manage care plans for patients with HIV, substance use, and behavioral health needs
- Collaborate with healthcare providers and support teams to ensure continuity of care
- Monitor patient progress and update care plans as needed
- Provide resource coordination and referrals to appropriate services
- Maintain accurate documentation and compliance with regulatory standards
- Support population health initiatives and outreach efforts
- Participate in team meetings and case reviews
Qualifications
- Copy of MSW degree - required
- Minimum 1 year of experience as a Licensed Master Social Worker
- Strong experience in HIV care management, substance use, and mental health
- Excellent organizational, communication, and coordination skills
- Ability to work independently in a hybrid work environment
- English proficiency required
Important Details:
- 13-week contract position
- Shift is 7 hrs/day (Mon - Fri, Shift time: 9 am - 5 pm), 35 hrs/week
- Location is New York, NY 10004
- Pay Range for Locals: $55 - $58 per hour
