Job Description:
- A minimum of two years of clinical experience in HIV care or support systems that includes experience in care coordination, health education and case management
- Work schedule: Monday to Friday
- Shift timing: 9:00 AM – 5:00 PM
- Hybrid work setup (remote + onsite)
- Non-patient-facing position
- Full-time role (35 hours per week)
Key Responsibilities:
- Manage care for patients with HIV
- Support individuals with substance use disorders
- Assist patients with mental health conditions
- Coordinate and monitor care plans
- Collaborate with healthcare teams for effective outcomes
- Maintain accurate documentation and records
Candidate Requirements:
- Master’s degree from an accredited college or university in Social Work is required
- Experience in HIV care management
- Experience working with substance use and mental health disorders
- Strong English communication skills
- A valid New York State license and current registration to practice as a Licensed Mental Health Counselor (LMHC), Licensed Master Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW) is required.
