General Summary
Provides quality clinical mental health assessments and consultation to providers and supporting clinical care team staff as necessary. They are responsible for triage and management of patients with clinical issues related to their behavioral or psychosocial wellbeing. Additionally, the behaviorist conducts psychosocial assessments, interventions, Transitions of Care (TOC), education, and referrals to inform a holistic treatment plan for patients, in support of Population Health. They are responsible for the management and oversight of a caseload of moderate - high risk Members with complex medical/behavioral /psychosocial needs.

Essential Functions:
- Conduct thorough biopsychosocial assessments to determine patient needs and appropriate interventions.
- Utilize standardized tools and clinical interviews to assess mental health conditions, substance abuse, and social determinants of health.
- Provide evidence-based individual, group, and family therapy to address mental health and behavioral issues.
- Develop and implement personalized care plans with measurable goals and objectives.
- Utilize therapeutic techniques such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Motivational Interviewing (MI).
- Coordinate care and services for patients, including referrals to community resources, follow-up, and continuity of care.
- Collaborate with multidisciplinary teams, including psychiatrists, psychologists, nurses, and primary care providers.
- Identifies, refers and advocates for patients requiring specialty behavioral health services and other services as needed.
- Maintain accurate and timely documentation of assessments, treatment plans, progress notes, and discharge summaries in compliance with regulatory and organizational standards.
- Ensure all practices comply with ethical standards, state licensure requirements, and HIPAA regulations.
- Provide education to patients and families about mental health conditions, treatment options, and coping strategies.
- Assists in the detection of “at risk” patients and the development of plans to prevent further psychological or physical deterioration.
