• Conducted comprehensive biopsychosocial assessments to develop individualized, evidence-based treatment plans and measurable therapeutic goals.

• Facilitated high-impact individual and group therapy sessions utilizing diverse modalities to address complex mental health and behavioral needs.

• Maintained meticulous, HIPAA-compliant clinical records, ensuring all observations and progress notes met rigorous professional and legal standards.

• Systematically monitored patient progress and intervention effectiveness, proactively adjusting clinical strategies to optimize recovery outcomes.

• Managed holistic care transitions by synchronizing efforts between families, medical providers, and community resources from intake through discharge.

Licensure / Certification / Registration:

• One or more of the following required:
  
  • Licensed Counselor specializing in Mental Health credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.
  • Licensed Counselor specializing in Clinical Addiction credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.
  • Licensed Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.
  • Licensed Therapist specializing in Marriage & Family credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Master's degree in Social Work, Counseling or related field required.
