License Clinical Social Worker - Addiction Medicine (Full Time, Day)

At NorthBay Health the LCSW provides direct clinical social work service, counseling, and case management services to Addiction Medicine patients including psychosocial assessment, development of a plan of care, implementation and documentation of emotional, psychological, social, social, social, financial, environmental, environmental or follow-up referral needs Works collaboratively with Addiction Medicine interdisciplinary members and other colleagues within the NorthBay Health. Assesses patients and develops a psychosocial plan of care within established timeline. Responds to referrals in a timely manner. Monitors progress toward goals of plan of care. Facilitates integration with community-based resources to optimize patient health. Collaborates with community partners to optimize patient health. Documents assessment and interventions in patient’s medical record. Activities are consistent with System mission and values and in compliance with applicable regulations and System policies and procedures. Works under the direction of the Medical Director of Addiction Medicine.

 

PRIMARY JOB DUTIES

  • Collaborates closely with the addiction medicine physician and interdisciplinary clinic staff to support patient care.
  • Assists with new patient intakes including psychosocial assessments and evaluation of substance use history.
  • Provides trauma‑informed psychotherapy for co‑occurring disorders such as depression, anxiety, and PTSD. EMDR training preferred.
  • Utilizes DSM‑5 diagnostic criteria and ASAM Levels of Care to support treatment planning for substance use disorders and behavioral addictions.
  • Provides short‑term or longer‑term therapy depending on clinical need and provider availability.
  • May develop and facilitate psychoeducational groups, group therapy sessions, or group medical visits related to addiction recovery.
  • Performs case management for patients with complex psychosocial needs including psychiatric illness, cognitivedisorders, housing instability, and chronic medical conditions.
  • Coordinates care with community partners including detoxification programs, residential rehabilitation programs, outpatient treatment centers, psychiatric providers, and community mental health agencies.
  • Assists with complex referrals and ensures successful transitions to higher levels of addiction treatment when indicated.
  • Requests, obtains, and reviews outside medical records from substance use treatment programs, psychiatric facilities, and other healthcare organizations when clinically appropriate.
  • Conducts patient outreach between scheduled appointments to support care coordination, answer questions, and facilitate follow‑up care.
  • Performs other duties as assigned.
