Evaluates social and emotional status of hospice and palliative care clients and families and provides direct short-term skilled social work intervention to clients and caregivers as needed to promote peaceful death and optimum family coping
Provides direct short-term skilled social work intervention service based on professional assessment of social and emotional status of client and family.
Assists with long term care planning, including Advanced Directives and placement for in-patient care.
Confers and consults with Agency staff regarding status and plan of care for client family.
Refers cases to other agencies for service as appropriate.
Prepares required documentation.
Maintains thorough knowledge of Agency services, policies and procedures, and community resources. HHC Values Integrity Caring Excellence Safety JOB Participates in case conferences, interdisciplinary team, and Agency staff meetings, conducts outreach activities for purposes of program marketing, community education, and collaboration with other service providers
Acts as liaison to other community services and agencies and to hospital and skilled nursing facilities collaborating on providing in-patient hospice care
Acts as resource for Agency staff regarding community resources.
Participates in health care programs as required.
Education: Master’s Degree in Social Work from a school accredited by the Council on Social Work Education required. LCSW required
Experience: One year experience in a health care setting required.
