Job Title: Hospice Registered Nurse (RN) Case Manager

Location: Fort Smith

Reports To: Director of Hospice

Position Summary: We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) Case Manager to provide essential care and support to patients in a home health setting. The RN Case Manager will coordinate and deliver comprehensive end of life care, ensuring patient comfort, symptom management, and support for patients and families using an interdisciplinary approach.


Key Responsibilities:

  • Patient Care: Perform admissions, intake information, and provide explanation of Hospice benefit/Medicare, complete physical assessments, and enter documentation into the Electronic Medical Record (EMR) system timely. Evaluate and perform ongoing assessments and revise initial Plan of Care in collaboration with Interdisciplinary Team (IDT) as the needs of the patient and/or family change.
  • Collaboration: Assist Medical Director and Hospice and Palliative Care Navigator in completion of initial comprehensive evaluations to determine hospice eligibility. Work with all disciplines of the Hospice team to ensure both patient and family needs are met.
  • Supervision: Provide supervision of Hospice LPN and Hospice Aides.
  • Education: Work with a team approach to coordinate community resources and other disciplines participating in patient care.
  • Compliance: Complies with all Agency policies, standards of practice, and operational guidelines.
  • Documentation: Maintain accurate, detailed documentation, reports and records in accordance with Agency and regulatory standards. Prepare IDT notes and participate in IDT meetings as scheduled.
  • On-Call Rotation: Participate in scheduled on-call shift rotation.
  • Other Duties: Perform additional tasks as assigned to support the care team and patients.

Requirements

  • Minimum of an Associate’s degree in Nursing. Bachelor of Science in Nursing (BSN) preferred.
  • Current Registered Nursing license from the Arkansas State Board of Nursing.
  • Home health care experience preferred.
  • Valid driver’s license.
  • Current CPR certificate.
  • Ability to travel out-of-the area and overnight.
