Job Title: Hospice & Palliative Care Navigator
Location: Fort Smith, AR

Position Summary:
We are seeking a compassionate and dedicated Hospice & Palliative Care Navigator to proactively identify and engage with potential palliative care and hospice care patients and their caregivers, ensuring clear and compassionate communication regarding available services and to provide comprehensive, accurate, and empathetic education to support patients and caregivers in making informed healthcare decisions. This position will collaborate closely with both our palliative and hospice care teams to enhance public awareness of these services while driving patient admissions to meet organizational goals. This position will facilitate the timely and seamless admissions process for patients who choose our hospice services, ensuring high-quality care delivery from the beginning.
Key Responsibilities:
· Referral Relationship Development: Build and maintain strong relationships with physicians, hospitals, social workers, ministers, senior centers, and other referral sources to increase awareness and generate patient referrals.
· Marketing, Outreach and Education: Develop and execute marketing and sales plans; conduct community and partner education events on palliative and hospice care programs, philosophy, advance directives, and available services to enhance public awareness of AAAWA programs. Provide clear, compassionate education to patients and caregivers on both hospice and palliative care options, scope of services, and benefits to support informed decision-making.
· Admissions Outreach and Appointment Management: Make outbound calls, schedule on-site appointments, and meet with patients and caregivers to guide them through the admissions process.
· Intake and Documentation Management: Facilitate timely, accurate intake, Start of Care, and admissions documentation, ensuring compliance with program guidelines and AAAWA policies.
· Care Coordination and Communication: Collaborate with hospice and palliative care teams to ensure smooth transitions of care and act as liaison between patients, families, and healthcare providers to support patient advocacy and satisfaction.
· In-Home Services: Assist the palliative care team with in-home visits as needed to support continuity of care. Offer emotional and information support throughout the hospice admissions and transition process to ensure patients and caregivers feel guided and understood.
· Reporting and Tracking: Monitor referral sources, track referral distribution, and report trends to inform and adjust the marketing/sales plan.
· Company Culture: Always project a professional image and attitude while representing AAAWA. Must wear agency-issued identification at all times while performing work duties.
· Other Duties: Perform additional responsibilities as assigned by supervisors or management to meet the needs of the patient and the team.

Requirements
1. Minimum of Bachelor of Science in Nursing (BSN), Social Work, Marketing or Business Management strongly preferred, or equivalent experience.
2. Current Registered Nursing license from the Arkansas State Board of Nursing.
3. 2+ years of sales/referral marketing experiences preferably in hospice, social work, or a related field.
4. Strong interpersonal and communication skills.
5. Must be detail oriented, able to prioritize and utilize effective time management skills and be flexible to adjust to impromptu situations on a regular basis.
6. Valid driver’s license.
7. Current CPR certificate.
8. Ability to travel out-of-the area and overnight.
