Role Description
This is a full-time, on-site role for a Hospice Nurse located in Denver, CO. The Hospice Nurse will provide comprehensive care to patients, focusing on pain management and palliative treatments. Responsibilities include conducting patient assessments, creating and implementing individualized care plans, offering emotional and physical support, and collaborating with interdisciplinary teams to provide holistic care. The role requires effective communication with patients, families, and other healthcare professionals to ensure a high standard of care and comfort.
Qualifications
- Proficiency in Nursing practices, including administering medications and performing clinical procedures
- Strong skills in Palliative Care, with a focus on pain and symptom management
- Current Registered Nurse (RN) license
- Previous experience in hospice or palliative care is strongly preferred
- Minimum of two years of experience as an RN in a hospice or related healthcare setting.
- Strong clinical assessment and critical thinking skills.
- Experience in Home Care settings, providing compassionate and personalized care
- Ability to deliver Direct Patient Care, ensuring high-quality treatment and support
- Understanding of Medicine, including medical terminology and healthcare best practices
- Exceptional communication and organizational skills
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver's license and reliable transportation for home visits.
- Current CPR certification.
- Knowledge of regulatory requirements and guidelines governing hospice care and documentation.
- Demonstrated commitment to providing compassionate, patient-centered care in a hospice setting.
- Experience with electronic medical record systems and documentation preferred.
Primary Responsibilities and Duties
- Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
- Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
- Coordinate and supervise the activities of hospice aides and other healthcare professionals involved in patient care.
- Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
- Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
- Collaborate with the interdisciplinary team to develop and implement patient care strategies that align with patients' wishes and goals.
- Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
- Provide bereavement support and resources to patients' families and caregivers as needed.
- Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
- Perform all other duties assigned by the Hospice Director or designated supervisor.
