Hospice Nurse

We are seeking a dedicated Registered Nurse (RN) with hospice experience to provide high-quality, patient-centered care in a home hospice setting. This role involves comprehensive patient assessments, care planning, case management, and collaboration with an interdisciplinary team. The ideal candidate will have prior GIP experience in a hospital setting, strong clinical judgment, and the ability to support patients and families through end-of-life care.


Key Responsibilities

  • Perform patient assessments and develop individualized hospice care plans
  • Manage and coordinate patient care as part of an interdisciplinary team
  • Monitor patient progress and adjust care plans as needed
  • Complete timely and accurate documentation using EMR systems
  • Provide education and support to patients and families
  • Ensure proper medication management and reconciliation
  • Maintain compliance with hospice regulations and insurance guidelines
  • Participate in IDT meetings, staff meetings, and quality improvement initiatives
  • Supervise home health aides (HHA) and ensure appropriate utilization of services
  • Maintain a safe environment and follow infection control protocols


Qualifications

  • Active Registered Nurse (RN) license in New Jersey
  • Associate Degree in Nursing (required)
  • Minimum 2–3 years of RN experience in a home hospice setting
  • Prior GIP experience in a hospital setting required
  • Valid New Jersey driver’s license
  • Basic Life Support (BLS) certification
  • Experience with EMR systems
  • Strong critical thinking, communication, and organizational skills


Important Details:

  • 13-week contract position
  • Shift is 5x7.5 Day shift (11 AM - 7 PM), 37.5 hrs/week
  • Location is Basking Ridge, NJ 07920
  • Hourly Pay Range: $55 - $58
