We are seeking a dedicated Registered Nurse (RN) with hospice experience to provide high-quality, patient-centered care in a home hospice setting. This role involves comprehensive patient assessments, care planning, case management, and collaboration with an interdisciplinary team. The ideal candidate will have prior GIP experience in a hospital setting, strong clinical judgment, and the ability to support patients and families through end-of-life care.
Key Responsibilities
- Perform patient assessments and develop individualized hospice care plans
- Manage and coordinate patient care as part of an interdisciplinary team
- Monitor patient progress and adjust care plans as needed
- Complete timely and accurate documentation using EMR systems
- Provide education and support to patients and families
- Ensure proper medication management and reconciliation
- Maintain compliance with hospice regulations and insurance guidelines
- Participate in IDT meetings, staff meetings, and quality improvement initiatives
- Supervise home health aides (HHA) and ensure appropriate utilization of services
- Maintain a safe environment and follow infection control protocols
Qualifications
- Active Registered Nurse (RN) license in New Jersey
- Associate Degree in Nursing (required)
- Minimum 2–3 years of RN experience in a home hospice setting
- Prior GIP experience in a hospital setting required
- Valid New Jersey driver’s license
- Basic Life Support (BLS) certification
- Experience with EMR systems
- Strong critical thinking, communication, and organizational skills

- 13-week contract position
- Shift is 5x7.5 Day shift (11 AM - 7 PM), 37.5 hrs/week

- Hourly Pay Range: $55 - $58
