Hospice Medical Social Worker - Peninsula

Sentara Health is seeking to hire a qualified individual to join our team as a Hospice Social Worker- MSW.

Position Status: Part Time, Days, Saturday and Sunday

Position Location: Greater Peninsula area of Virginia (clinician will need to be available to cover Newport News/Hampton up to Gloucester)

Pay Range: $25.60 - $42

Overview

Performs a variety of casework and provides social work services to patients, families, and significant Must develop and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses. Interviewing techniques; Skills for conducting group sessions.

Education

• MLD- Master's Level Degree in Social Work

Certification/Licensure

• BLS (Required within 90 days)
• Valid Driver's License

Experience

• Hospice, Home Health, Social Work, Group Social/Rec Program (previous experience required)
