A Hospice Licensed Vocational Nurse (LVN) plays a vital role in delivering compassionate, patient-centered care to individuals at the end of life. In this role, you will provide skilled nursing support, manage symptoms, administer medications, and collaborate closely with an interdisciplinary team to ensure patients remain comfortable and dignified. You will also offer emotional support and guidance to families during a deeply meaningful time, helping them navigate care needs with empathy, respect, and professionalism.

As a Hospice Licensed Vocational Nurses (LVN), here’s a big-picture view of what you’ll do:

  • Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life
  • Work as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
  • Perform nursing services which are planned, delegated, and supervised by the registered nurse
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Take a holistic approach to care that engages the patient’s mind, body, spirit and family
  • Travel throughout the full service area is required for this role. 

 

A few must-haves for Hospice Licensed Vocational Nurses (LVNs):

  • Graduate of an accredited nursing program and active Licensed Vocational Nurse (LVN) license in California
  • Minimum of 1 year of hospice nursing experience
  • CPR certification, valid driver's license, reliable transportation, and valid auto insurance policy
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills and compassion
