Home Health Registered Nurse (RN)

Location: Newton Center, Massachusetts 02459

Pay Range: $65–$70 per hour

Job Summary

We are seeking an experienced and dedicated Home Health Registered Nurse (RN) to provide high‑quality nursing care in the home setting. The RN will be responsible for assessing patient conditions, implementing individualized care plans, and delivering ongoing education and support to patients and their families. Care is delivered in accordance with organizational philosophy, policies, and standards of community health practice.

This role also includes performing admission processes and case management responsibilities in alignment with MGB Home Care policies and standards of practice, utilizing accurate and timely electronic documentation and effective interdisciplinary communication.

Key Responsibilities

• Conduct comprehensive patient assessments in the home environment
• Develop, implement, and evaluate individualized plans of care
• Provide patient and family education and ongoing support
• Perform admissions and manage assigned patient cases
• Coordinate care with physicians and interdisciplinary team members
• Maintain thorough, accurate, and timely electronic documentation
• Communicate changes in patient condition promptly and effectively
• Adhere to all organizational policies and standards of practice
• Participate in weekend rotation once every 6 weeks

Required Qualifications

• Minimum 1 year of Certified Home Care experience (required)
• Minimum 1 year of Case Management experience (required)
• Active RN license in good standing
• Ability to travel to assigned patient cases
• Strong clinical assessment and critical‑thinking skills
• Proficiency with electronic health record (EHR) systems
• Excellent communication and organizational skills

Schedule & Travel

• Weekday caseload with weekend rotation every 6 weeks
• Must be able to travel to patient homes as assigned

*Offered pay rate will be based on education, experience, and healthcare credentials.
