Supportive Home Health Care is expanding rapidly across Southern California and seeking an experienced, results‑driven Home Health Liaison / Marketer with a strong Medicare referral background and an active book of sales. This is a high‑impact role for a seasoned professional who excels in relationship development, referral growth, and strategic outreach within the healthcare community.

About the Role

As a key connector between our home health program and the broader healthcare network, you will:

  • Build and strengthen referral relationships
  • Drive Medicare patient referrals
  • Advocate for patients needing coordinated, compassionate in‑home care
  • Collaborate with clinical teams to ensure smooth transitions of care
  • Support outstanding patient outcomes and agency growth

You will represent Supportive Home Health Care with professionalism, empathy, and integrity while expanding our presence across Southern California.


Key Responsibilities

  • Cultivate and maintain strong partnerships with:
  • Physicians & physician groups
  • Hospitals & discharge planners
  • Skilled Nursing Facilities (SNFs)
  • Case managers & community healthcare providers
  • Drive timely and appropriate Medicare referrals
  • Educate providers, patients, and families on home health services and eligibility
  • Assess patient needs and coordinate care plans with nursing and therapy teams
  • Ensure smooth transitions from hospital or facility to home
  • Track referral activity, growth metrics, and territory performance
  • Maintain a consistent field presence to strengthen referral pipelines and expand market reach

Required Qualifications

  • Active book of sales (mandatory)
  • Minimum 2 years of recent experience as a Home Health Liaison or in home‑health‑specific marketing/referral development
  • Current or very recent employment in a healthcare organization performing this role
  • Demonstrated Medicare referral success
  • Proven track record in healthcare sales, outreach, or referral growth
  • Strong knowledge of Medicare home health eligibility
  • Exceptional communication, relationship‑building, and presentation skills
  • Excellent time management, organization, and decision‑making abilities
  • Ability to manage a territory independently and effectively
