Basic Job Function Summary
The Home Care Liaison is responsible for refining and improving the transition processes across all sites of home and hospice care. This role manages the efficient and effective facilitation of referrals to the post‑acute homecare/hospice agency, educates acute care staff (including physicians) about available homecare/hospice services, and may participate in business development activities related to growth or new program expansion.
