We are seeking an experienced and detail-oriented Health Home Supervisor to lead our care management team and support the delivery of coordinated, patient-centered services.
Position Overview
The Health Home Supervisor oversees a team of care managers and ensures the effective delivery of Health Home services in compliance with NYS DOH and Medicaid program requirements. This role focuses on staff supervision, care coordination oversight, quality assurance, and program performance.
Key Responsibilities
- Supervise and support Health Home Care Managers and care coordination staff
- Monitor staff caseloads, productivity, and documentation to ensure compliance with program requirements
- Provide coaching, training, and performance feedback to team members
- Conduct regular case reviews to ensure appropriate care planning and follow-up
- Ensure timely completion of assessments, care plans, and service documentation
- Coordinate with healthcare providers, behavioral health specialists, and community organizations
- Track program metrics and quality indicators to meet performance goals
- Address escalated patient issues and assist with complex cases
- Maintain compliance with HIPAA regulations, Health Home standards, and organizational policies
- Participate in team meetings, audits, and quality improvement initiatives
- Conduct routine documentation and quality reviews to ensure compliance with NYS Health Home standards and Medicaid billing requirements.
- Monitor program performance metrics including engagement, assessments, and care plan completion, and provide coaching to staff to improve outcomes.
- Provide supervision and support to field-based care managers, including guidance on complex cases and member engagement strategies.
- Support program leadership with audit readiness, workflow improvements, and implementation of care management systems (e.g., Foothold, MAPP, or similar platforms).
Qualifications (per NYS DOH requirements)
- Master’s degree required in Social Work, Nursing, Public Health, Healthcare Administration, or a related field
- 2–3 years of experience in care management, case management, or healthcare coordination
- 1–2 years of supervisory or leadership experience preferred
- Knowledge of Medicaid Health Home programs and care coordination models
- Strong leadership, organizational, and communication skills
- Experience with Electronic Medical Records (EMR) and care management platforms
- Experience with NYS Health Home programs and care management platforms such as Foothold (FCM), PSYCKES, MAPP, etc. preferred.
- Experience supervising field-based and remote care management staff.
- Experience coordinating community-based services and benefits (e.g., public assistance, Medicaid recertification, housing, and food resources) to address social determinants of health.
