The Avalon of Lewis Center is recruiting skilled, compassionate, detailed-oriented LPNs or Certified Med Tech to join our team! This position provides direct nursing care to residents and delegates and supervises the care provided by our Quality-of-Life Specialists/Resident Assistants.

Licensed Practical Nurse (LPN) 7a-7p/ 7p-7a – AL/MC Responsibilities
- Provide day-to-day clinical oversight and guidance to caregiving staff, ensuring quality resident care and compliance with community policies.
- Monitor residents for medical, functional, and behavioral changes; promptly communicate findings and assist with updating and implementing individualized service plans.
- Safely administer prescribed medications and treatments; oversee medication management processes and ensure continuity of nursing care.
- Assess, monitor, and support residents with chronic and acute medical conditions in collaboration with physicians and other healthcare providers.
- Respond appropriately to changes in residents’ medical status, including initiating interventions and coordinating outside services when necessary.
- Participate in the admission, orientation, and ongoing evaluation of new residents to ensure a smooth transition into the AL/MC community.
- Support the Director of Health Services (DOHS) with resident assessments, care evaluations, and ongoing monitoring of resident health and safety.
- Maintain accurate and timely clinical documentation in accordance with regulatory standards and community protocols.
Here are a few of the qualifications we need you to have
- Current nursing license valid in state worked
- Ability to speak, read and comprehend English language
- Good Communication and teamwork skills
Salary Range for LPN - $30.00-$32.00
Shift differential for evenings and weekends!
$1000 Sign on bonus offered for full time workers!
