Program Coordinator - Family Support Services Position DescriptionProgram Areas: BRICK, Family Care Plan (FCP)Supervisor: Director of Family Support Services                                                                              Supervisees: Family Peer Support Workers, DoulasPosition SummaryThe Program Coordinator Family Support Services is responsible for the day-to-day coordination and implementation of SIRACs BRICK and Family Care Plan programs. This role ensures that families receive timely, high-quality, trauma-informed support while maintaining strong program organization, staff oversight, and communication across team members and partners. The Program Coordinator provides direct supervision and support to Family Peer Support Workers and Doulas, ensuring services are delivered with consistency, quality, and alignment to program goals. This position serves as the operational lead for service deliverymanaging workflows, supporting staff performance, and maintaining compliance in a rural, multi-county service area.KEY RESPONSIBILITIESProgram Coordination & Service DeliveryCoordinate daily operations of BRICK and Family Care Plan programming, ensuring services are organized, responsive, and aligned with program goals.Manage incoming referrals, assign cases, and monitor participant engagement and service timelines.Ensure continuity of care through consistent communication with families, staff, and partners.Identify and address barriers to service access and implement real-time solutions.Other duties as assigned.Staff Supervision & SupportProvide direct supervision to Peer Support Workers and Doulas, including regular 1:1 meetings, performance feedback, and coaching.Monitor caseloads, productivity, and service quality to ensure alignment with program expectations and funder requirements.Review case documentation for accuracy, completeness, and timeliness.Support staff in navigating complex family situations through case consultation and problem-solving.Facilitate team meetings, case reviews, and ongoing skill development opportunities.Assist in onboarding and training new staff on program workflows, expectations, and tools (e.g., SIRAC Connect).Documentation, Data & ReportingEnsure timely and accurate documentation of all services in SIRAC Connect and other required systems.Track program metrics (enrollment, service delivery, outcomes) and proactively address gaps.Prepare or support monthly reports, funder deliverables, and internal data reviews.Maintain organized records to support audits, monitoring visits, and evaluations.Quality Assurance & ComplianceConduct routine audits of case files and service documentation to ensure compliance with funder and organizational standards.Reinforce adherence to program policies, workflows, and service models.Participate in continuous quality improvement processes, including identifying trends and implementing corrective actions.Ensure services are delivered in alignment with trauma-informed, family-centered, and recovery-oriented principles.Partnership Coordination & CommunicationMaintain active communication with referral partners including hospitals, DCFS, behavioral health providers, and community-based organizations.Coordinate referrals and service linkages to ensure families are connected to appropriate supports.Support maintenance of the community resource directory and referral pathways.Participate in case coordination meetings or cross-system collaboration as needed.Family Engagement & Program SupportPromote a welcoming, non-stigmatizing, and family-centered approach across all program activities.Support engagement strategies that build trust and sustain participation.Assist in gathering family feedback and elevating participant voice to improve services.QUALIFICATIONSBachelors degree in Social Work, Human Services, Public Health, or related field (preferred); relevant professional experience may be considered in lieu of degree.Minimum of 3 years of experience in social services, behavioral health, or community-based programming.Prior experience supervising staff or providing team leadership strongly preferred.Experience working with families impacted by substance use, mental health, or child welfare systems preferred.Strong organizational and time management skills, with the ability to manage multiple staff and priorities.Experience with documentation systems and data tracking.Strong communication, coaching, and problem-solving skills.Commitment to trauma-informed care, harm reduction, and family-centered practices.WORK ENVIRONMENT:The work environment may include a combination of office-based, community-based, and possible remote work dependent on programming needs.
