JOB DESCRIPTION POSITION TITLE: Case ManagerREPORTS TO: Services Manager LOCATION OF POSITION:  1551 W Armory WayHOURS:   Day and Swing shifts available.    STATUS: Full Time                                                   Salary: 33.65/hr.  Agency Description:    Everyone Deserves Housing (EDH) exists to ensure that people experiencing street level homelessness have a clear, compassionate pathway off the streets and into safe, stable housing. We work to break down barriers to services, recognizing that lasting stability requires equitable access to housing, healthcare, and community-based supports.  Our work aims to provide people with the healthcare and support they need through emergency shelter and housing programs. The team promotes social change to address systemic racism and inequities linked to housing instability. We use a trauma-informed approach with clients and colleagues, recognizing how trauma impacts lives.      Position Description:  The Case Manager provides engagement and supportive services using a harm reduction approach to stigmatized populations which may include people who have substance use disorders, engagement with child welfare systems, have significant mental health diagnoses or physical health issues.Our programs include 24/7 enhanced shelter and scattered sites housing stabilization programs. The emphasis is on meeting individuals where they are at and providing non-judgmental, non-coercive services and resources to individuals with a focus on housing stabilization. The Case Manager will work directly with participants to obtain/maintain stabilized permanent housing and to develop an individualized stability plan.Case Managers will work closely with various providers, support services, and community resources.   Key Responsibilities:  Provide presence and staffing in the shelter environment and respond to client needs as appropriate.  Work with clients to identify barriers to create and document a housing stability plan in the case management database.  Build rapport, provide support and advocate for a diverse range of clients including individuals who are actively using substances and/or are living with significant mental/physical health challenges using harm reduction approaches when applicable.  Participate in verbal de-escalation and other interventions in emergent situations and be willing and able to assist other staff as needed to maintain a safe and secure environment. Provide support and mediate disputes between clients. Adhere to and follow program specific guidelines and regulations.  Assist clients in mitigating behaviors that could adversely affect their ability to continue in the program by participating in care coordination and care review meetings.  Assist clients in obtaining documentation needed to obtain/maintain housing and accompany clients to outside appointments as needed.  Meet with clients on an ongoing basis to ensure that they are making progress on their goals. Provide clients with referrals to other community resources to resolve barriers to housing, and develop and maintain cooperative relationships with current programs providing services. Assist clients in moving into/maintaining their housing units as needed. Ensure the client documentation aligns with agency and program standards.  Attend staff meetings, workshops, retreats, community meetings and in-service training as provided. Maintain accurate record of hours worked and turn in timesheets on schedule. Contribute to and support a positive, team-oriented work environment; participate with other staff members in group decision-making process. Follow established credit card policy for any purchases. Work in-person at a program and attend required in-person agency meetings. Other Duties as assigned.  Job Conditions:  This position requires the employee to work in an environment where there may be exposure to illicit drug residues and fumes or other bio-hazardous materials in the course of doing business.  In addition, there is potential for exposure to bloodborne pathogens. Other exposures could include cleaning supplies, chemicals involved in pest control, paint and other materials used in building maintenance, building temperature fluctuations, dust, noise, pests, and odors.Other working conditions may include interruptions, working alone, evening or weekend work, responding to emergencies and working on-call.  This position also requires the employee to work with clients who may have untreated or poorly treated substance use and mental health disorders.  These clients may demonstrate challenging behaviors that require staff support for de-escalation or redirection.  Physical Requirements: The physical demands described here are representative of those an employee may encounter while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Occasionally positions self to assist with shelter setup, cleanliness, and organization. Moves throughout the facility to monitor client areas, including outdoor gravel pathways and indoor common spaces. Occasionally lifts or moves items such as bedding, food items, or supplies, generally up to 25 pounds, but occasionally up to 40 pounds. Communicates effectively with clients, coworkers, volunteers, and emergency personnel in person and through written or electronic means. Works in an environment that may include exposure to emotionally charged situations, crisis response, or unpredictable client behavior.  Minimum Qualifications Associates degree in social work, social services or related field or equivalent work experience or credentialing such as a certified peer counselor.  One to two years experience performing work related to housing and/or employment assistance. Must be able to attend required meetings and trainings. Understanding of problems homeless individuals and families often face, including alcohol and drug abuse, mental illness, trauma, domestic violence, and long-term homelessness. Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations. Proficiency with Microsoft offices suite, strong written and oral communication skills and ability to quickly learn the client services database. Ability to work independently and as a member of a staff team. Must have the capacity to take initiative to ensure job duties and client outcome expectations are met. Willingness to learn and work within a diverse environment. Criminal history background checks are required prior to employment. Commitment to harm reduction program model.  Access to reliable transportation to travel throughout King County as needed for the program. Preferred Qualifications Bachelor's degree in social work, social services or related field. Valid WA State Driver's License and ability to meet EDH Driving Policy requirements. Knowledge of the local subsidized housing and low-rent private housing market. Relevant experience with high-acuity and/or chronically unhoused individuals. Secondary language proficiency. EDH IS AN EQUAL OPPORTUNITY EMPLOYER AND VALUES DIVERSITY IN THE WORKPLACE
